GOLDEN ACCESS PASS (for citizens or permanent residents of the United States who are blind or permanently disabled)
The Golden Access Passport is a lifetime entrance pass to national parks, monuments, historic sites, recreation areas, and national wildlife refuges that charge an entrance fee. The Golden Access Passport admits the pass signee and any accompanying passengers in a private* vehicle if a park has a per vehicle entrance fee. Where a per person entrance fee is charged, the Golden Access Passport admits the pass signee, spouse, and children.

IHC CLINICAL GENETICS INSTITUTE - Marc S. Williams, MD, FAAP, FACMG

Dr. Marc Williams graduated from the University of Wisconsin-Madison with a BS in Chemistry and an MD. He did a pediatric residency at the University of Utah from 1981-1984. He received board certification in Clinical Genetics in 1996. In 1999, he gave up general pediatric practice and became the associate medical director of the Gundersen Lutheran Health Plan while maintaining his genetic practice. It was by combining these two areas of interest that he developed an interest in the role of genetics in health care delivery. In January of 2005, he became the director of the Intermountain Health Care Clinical Genetics Institute. The IHC Clinical Genetics Institute was created to manage clinically applicable genetic advances, develop new genetic intervention protocols, and learn how to apply genetic information to disease prevention and treatment. In addition to his administrative duties, Dr. Williams runs a clinic for evaluation of adults with mental retardation, birth defects, and genetic disorders. He is currently chair of the Committee on the Economics of Genetic Services of the American College of Medical genetics, as well as the subcommittee on Health Care Systems of the Section on Genetics of the American Academy of Pediatrics. He is the Editor-in-Chief of the Manual on Reimbursement for Medical Genetic Services. In his abundant free time, Dr. Williams is an accomplished singer (Bass Section Leader, Utah Symphony Chorus) and trombonist (Orchestra of Sandy). He also owns an art business specializing in late 19th and early 20th century painting and prints. Office number is (801) 408-8956.


UTAH MEDHOME PORTAL

This is the Utah Collaborative Medical Home Project site. Information, Tools, and Resources to aid Primary Care Physicians in caring for Children with Special Health Care Needs (CSHCN) and providing a Medical Home for all of their patients.


SALT LAKE ADVOCACY and COMMUNITY TRAINING (Salt Lake ACT)

Our Service Brokers are professionals and paraprofessionals. Together we have fifty years of experience with people who have developmental disabilities and traumatic brain injuries. Salt Lake ACT collaborates with dozens of community agencies to help link people with community services. Salt Lake ACT can provide services in Spanish, French TO Salt Lake, Tooele, and Summit counties.


UTAH’S HOME and COMMUNTIY-BASED WAIVER PPRGRAM for TECHNOLOGY DEPENDENT CHILDREN - "Travis C. Waiver"

Under "waivers" of specific (statutory) comparability and eligibility requirements, this program provides unique home and community-based services to a limited number of eligible individuals who would otherwise require placement in a certified nursing facility to receive Medicaid coverage for their care and services.

UDSF State Activities & News -

Even though we got wet a great time was had by all at the 2008 “Down the Fairway” Golf Tournament which was held May 29th at Thanksgiving Point. Highlights as always included pictures with the golfers and our very special helpers, driving around in golf carts, dancing, and French fries from the café. The really fun thing this year was our Trisomy 21 Hole where we had our kids tee off for the golfers. If the kids hit the balls certain distances the golfers could take up to 3 strokes off! The kids and fun and the golfers were thrilled!!! To see more pictures click here.

The Spotlight is On: UDSF Dagget, Duchesne, Uintah Chapter President - Wendy Strebel

Leona Cooper is one of those moms that seem to do it all! She is involved with all of her children’s activities from football and wrestling to dance. You can tell where her team spirit lies as her yard and car proudly displays her children’s school mascot and team number. Sydney is the youngest of her 4 children and is a treasure, but I can guarantee that the rest of her children are also treasures! You can see it in the way they treat Sydney. Leona keeps things going in the Davis chapter, but she has learned to delegate very well. Several of the Davis activities have been planned in part by a boy scout wanting to earn his Eagle Scout Award. Leona has volunteered for the UDSF for many years. She is greatly valued and appreciated!

Happy Birthday Spotlight!

Look up the word amazing and most likely you will find Dawn! Dawn will turn 45 on June 22nd. She is right in the middle of 7 siblings; she has 2 brothers and 5 sisters. One sister has passed away. Dawn was born and raised in Tooele. Her father, who Dawn admires “for everything he has done for me” started a school in Tooele county for individuals with special needs when Dawn was going to school. Dawn said her father is “more my hero than anything.”
Dawn has been working since before she was 18. Some of the jobs she has had have been working in a bakery’s, bagging grocery’s, housekeeping, working in restaurants, and her most recent is working at Cinemark Sugarhouse Movies 10. The most fun she had working was at Detroit Diesel where she cleaned parts for trucks and did inventory.

Volunteering is big on Dawn’s list of things to do. Currently Dawn volunteers for Best Buddies Utah. She has also sat on CAT (Committee on Accessible Transportation) for the UTA (Utah Transit Authority). Dawn gets all over the city on UTA and has been doing so for many years. When she is not busy working or volunteering she enjoys going to the movies and working on her computer. She also has found time to be the secretary for the apartment complex where she lives…on her own! If she gets a chance to travel she likes to visit her dad in Florida.

Here at the UDSF we are happy to call Dawn our friend. She is a delight to have around. Dawn is scheduled to have surgery to remove her gall bladder and we want her to know that she is in our thoughts and prayers. We are wishing her a speedy recovery so she can have a great birthday with lots of cake and ice cream!
